In consideration of being permitted to participate in any way, including travel to and from, the Bay Area Lumberjack Competition, an American Lumberjack Association sanctioned competition, I hereby:

Acknowledge that I am familiar with the sport of lumberjack competition and understand the rules governing this competition.

Agree that, prior to participating, I will inspect the competition equipment, competition wood, and facilities, and if I believe anything is unsafe or beyond my capability, I will immediately advise a competition official of such conditions and refuse to participate.

Acknowledge and fully understand that I will be engaging in a sport that might result in serious injury, including permanent disability or death, and severe social and economic losses due not only to my own actions, inactions or negligence, but also to the actions, inactions, or negligence of others, the rules of the sport of lumberjack competition, or conditions of the premises or of any equipment used.  Further, I acknowledge that there may be other risks not known to me or not reasonably foreseeable at this time.

Knowing the risks involved in the sport of lumberjack competition, I assume all such risks and accept personal responsibility for the damages following such injury, permanent disability, or death.

Release, waive, discharge and covenant not to sue the American Lumberjack Association, Douglas Timber Operators, The Bay Area Chamber of Commerce, The Mill Casino · Hotel, together with their respective administrators, directors, agents, and other employees or volunteers of the Bay Area Lumberjack Competition committee, event officials, medical personnel, other participants, supervisors, sponsoring agencies, sponsors, advertisers, all of whom are hereinafter referred to as “Releasees”, from any and all claims, demands, losses, or damages on account of injury, including permanent disability and death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise to the fullest extent permitted by law.

I agree to allow The Mill Casino • Hotel to use and publish my name/likenesses, audio, video, print and film recordings of me for advertising, promotion or other commercial purposes.  I waive any right to inspect or approve the finished product, advertising or other copy and to all phases of its use in whatever form to be determined by The Mill Casino • Hotel and in its entities.  It is understood that no compensation will be paid by The Mill Casino • Hotel for the right to photo or re-use the audio, video, print and film recordings for promotional purposes.
